-APPOINTMENT DEPOSIT AND PAYMENT: When booking any service, we reserve a considerable amount of time designated specifically for you. We require a $100 deposit in order to reserve this time for your appointment. The deposit will be credited toward the procedure fees.
*Administration fee of 3.5% will be charged to booking deposit. Please pay the remaining balance the day of your appointment with Cash, Zelle ( bank transfer), or Apple pay to avoid the fee.
-IF YOU SHOW UP WITH: Any skin problem in or close around the area of the procedure like acne, cuts, cold sores, irritated skin, sun tanned skin, redness, cold sore, swelling, etc, we will not be able to perform the procedure and your deposit will be non refundable and we will require a new $100 deposit for the new rescheduled appointment.
-TARDINESS: Please, arrive to your appointment 15 minutes prior to the start of your appointment. You will need to complete a consent and medical intake form. If you are late, it compromises the amount of time the technician has to spend on your procedure, which in turn compromises your results.
In the event that the client is more than 15 min late, the procedure will be canceled and the deposit will not be refunded as we are not able to sell this time to another client.
-ASSIGNED PROCEDURE TIME: The time stated on the service is the typical we need to do the procedure including numbing of the area if your service requires it. We have the best numbing creams available and try to make your experience as comfortable as we can but please understand we cannot use more time to re apply numbing creams over and over and lose time for the procedure. If you want us to stop in the middle of the procedure, you understand you will still have to make the full remaining payment and you will need to reschedule and pay again as first visit. This will apply to any procedure of the services we offer.
-CANCELLATION POLICY: If you need to cancel or reschedule, we require the cancellation to be made at least 48 hrs prior to the start of your appointment time. If your appointment is canceled or rescheduled with less than 48 hrs( or in the case of a no-show), the deposit or full amount paid will not be refunded. Please note, If you do not call to cancel or do not show to your appointment, we will not re-book you.
-FOLLOW UP APPOINTMENTS: Additional touch ups might be needed after the first procedure and there will be an additional charge for that service. Some procedures include 1 follow up touch up for free . You still need to set the appointment the day of your first visit or online exactly between the time the Technician recommends for better results. If you try to schedule your complimentary touch up appointment after the time recommended, your first complimentary touch up (if included) will be lost and you will need to pay for the first touch up appointment. Tardiness and cancellation policies apply to any type of appointment.
-CLIENT ONLY IN THE PROCEDURE ROOM: Though a friend or a family member may accompany you to your appointment, we have learned they tend to be a distraction for both the client and the technician, but they are welcome to stay outside the procedure room on the waiting area. It is very important for the technician to completely focus on the procedure in order to have the best results possible. NO CHILDREN, TODDLERS OR BABIES: Boost Skin and Body LLC must kindly ask you to not bring them to your appointment. Thank you for your understanding.
-WE WILL NOT PERFORM ANY PERMANENT MAKEUP PROCEDURE IF: You are under age of 18 years old for Permanent Makeup, (any other procedure, we require a parent present and signed forms of consent.) If yo are prengant, If you have used Accutane for the past year, if you are under prescribed treatment for acne cream. If you use tanning beds or had a recent trip to the beach and got tanned, please allow at least a month after your tanning to be able to get the procedure. If you get botox treatments, please allow at least 2 weeks after your last botox to get the permanent makeup procedure. If you use blood thinning medication. We can perform 72 hrs after your last medication. Ask your Doctor if you can stop taking it for that period of time to get the permanent makeup procedure. We will not perform any procedure if you have cold or flu. Please reschedule if you still have symptoms.
-PLEASE BRING A VALID DRIVERS LICENSE OR STATE ISSUED ID: A copy will be made for our records to keep with your consent forms.
-PHOTOGRAPHY AND VIDEO RECORDING: At Boost Skin and Body, we require before and after pictures to be taken and some cases recording video of the procedure. An acceptance signed form of consent is necessary to be able to perform any procedure. If you do not wish to be recorded or have pictures taken, please do not book. We will not use your name on any picture or video. We might use pictures of the area treated for our album and social media but we will try our best to keep your identity safe. We care about the safety of our staff and clients so recording 24/7 is live at our facility including in our procedure rooms. If you love that we are a very safe facility, Please Book!
-IMPORTANT: When you book, you will receive a confirmation email and a Pre Procedure form. Please Read. In this form you will find important information that will let you know if you are a good candidate for the procedure. If you are not, please contact us asap to reschedule or cancel so you do not lose your deposit.
-Gift cards and eGIFT CARDS: Our egift cards are valid for Boost Skin & Body LLC locations with appointment only. Can be used toward any service and/or for purchase of any products we offer at our locations. eGift cards are not redeemable for cash. eGift cards are not transferable, we cannot print them and cannot not be redeemed for cash or used to purchase another gift card. We do not give cash balance back. Service prices are subject to change. Boost Skin & Body LLC is not responsible for lost, unused or stolen cards. eGift cards are valid for 12 months after issue date.
Boost Skin and Body LLC reserves the right to refuse to do any service if there is any safety concern for either the technician or the client.
